PRIVACY:
Your privacy is respected by us. The information you provide on this form will be used to provide services for you and for the
purposes of registration, team selection and insurance. It will be passed onto the Australian Baseball Federation (ABF) and to
the ABF’s insurer.
We and the ABF may wish to provide you with special offers from time to time. If you do not wish to receive this additional
information, please tick this box.
If you do not provide the information we may not be able to register you as a member.
We and the ABF comply with the Privacy Act with respect to the collection, storage and security of your personal details.
If you have any privacy concerns or would like to verify any information we hold about you, please contact our club registrar or
the ABF’s Privacy Officer.
You also give permission for Rouse Hill Baseball Club to publish photographs taken during matches and club functions to be
published online at www.wildcatsbaseball.com.au and understand that our preferred method of contact may be via email. As such
you authorise us to contact you via the email address provided on this form.
RISK WARNING:
You should be aware that there are risks of injury associated with playing baseball, as there are with most sports. Risks will arise
in the context of the activities of batting, pitching, catching and running. While we aim to minimise risks, it is not
possible to eliminate them all. As such by signing this form you acknowledge that you will not hold Rouse Hill Baseball Club or
any of its members or officials responsible for any injury or illness sustained. You also give express permission for Rouse Hill
Baseball Club to arrange for emergency transport should an injury be sustained that requires emergency transport and
acknowledge that you will pay any associated costs arising from this transport.
PLAYER CODE OF CONDUCT:
As an individual and team member, I will: 1. Work hard for my team and myself at all times. I will co-operate with my own team officials and with team mates and
opponents and obey any reasonable request from my team coaches or manager. 2. Play by the rules of the competition and follow all recommendations of The League regarding safe play and equipment
and obey the rules prohibiting banned substances. 3. Never argue with an umpire and always accept the decisions of the umpires as being fair and called to the best of their
ability. 4. Control my temper. I recognise that swearing or verbal abuse of umpires, team officials or other players will not be
tolerated. I will not abuse equipment (mine or anyone else’s). I will not throw bats, helmets or any other equipment. 5. Not cause any loss or damage to private property or cause a disturbance to others. 6. Not invite anyone other than team personnel into restricted areas such as the dugout. 7. Behave in a polite and appropriate manner toward others. I will applaud all good plays from either team and treat all
players as I would like to be treated. I will not interfere with, bully or take unfair advantage of any other player, nor
deliberately distract or provoke an opponent. 8. Be frank and honest with my coach concerning illness or injury and my ability to take part in training session or games. 9. Be responsible for my personal appearance, behaviour and personal hygiene and cleaning of my own equipment. I will
be neat and tidy in appearance at games, training sessions and at any time that I am in public as a baseball player. 10. Be prompt for meetings, training sessions and games. If I can’t attend on time (or at all) then I will call my team manager
or coach and say so, as soon as I can.
Should you not agree with any of these “code of conduct” conditions it is unfortunate that we will not be able to
accept your registration.
